In this role, you\’ll work closely with a dedicated team to deliver person-centered care, follow the Scottish Social Services Council (SSSC) Codes of Practice, and adhere to Hillcrest Futures Policies and Procedures. You\’ll play a key part in delivering positive outcomes for individuals while contributing to the development and improvement of our services.
Key Responsibilities:

• Provide direct support to service users in line with their Personal Outcome Plans.
• Undertake case coordination duties, advocating for the needs and interests of service users.
• Work effectively as part of a collaborative team.
• Communicate clearly and effectively with service users and colleagues.
• Contribute to team learning and service improvements.
• Continue developing your skills to meet SSSC registration requirements.
• Carry out housing management tasks as required by the service.
• Participate in varied shift patterns, including evenings, weekends, waking nights, sleepovers, and lone working.
