Our Home From Hospital Team is here to make life a little easier for people returning home after a hospital stay. We work closely with local health and social care services to support smooth, timely discharges and help prevent unnecessary readmissions. By connecting people with the right community services, we play a big part in reducing pressure on the wider system and making sure our customers feel safe and supported.
In this role, you’ll be the friendly face helping patients and their carers with the practical, non‑medical support they need to settle back home confidently. This might include offering advice, guidance, and signposting, as well as arranging practical help such as equipment, minor adaptations, key‑safe installation or care alarm systems. You’ll also support people with benefit claims, managing debt, or housing concerns – all the things that help promote independence and boost wellbeing.
We support Weston General, BRI & Southmead along with other nearby hospitals for residents returning home to North Somerset. You’ll be part of a rota visiting the different sites we work with, so being able to travel independently out of the immediate area is important.
About you
You’ll bring experience from a support, health, or care setting, along with a real passion for helping people. You’ll be comfortable working with a wide range of individuals and able to offer excellent customer service every single time. Strong communication skills, both written and verbal, are essential.
Just to note: this is not a domiciliary or personal care position. From time to time, you may need to be flexible with your hours to best support our customers. As you’ll be travelling across North Somerset communities and into Bristol, you’ll need to be able to drive and have access to your own vehicle (or reliable alternative transport). The successful candidate will also be subject to a satisfactory Enhanced DBS check and hospital security check.
