The Neighbourhood Alliance in Sussex are looking for dynamic, collaborative individuals to join us as Neighbourhood Integrators, supporting the development of Integrated Community Teams (ICTs) across Sussex. This is an exciting opportunity to play a key role in delivering the NHS Neighbourhood Health Plan by helping partners work together to provide more joined-up, community-based care closer to home.

Working at the heart of a local ICT, you will bring together organisations, teams, data and insight to strengthen collaboration, improve outcomes and reduce health inequalities. You will work across organisational boundaries, influence system change, and help build more connected, responsive services for local communities. There are 15 Integrated Community Teams (ICTs) across Sussex, with an ambition to appoint a dedicated Neighbourhood Integrator to each. This recruitment represents the first phase of appointments, with further opportunities expected as the programme develops.

Main duties of the job

This role is hosted by the Primary Care Provider Collaborative delivery arm, one of the core partners within the Sussex Neighbourhood Alliance. The Neighbourhood Alliance brings together NHS organisations, local authorities, social care and voluntary sector partners to deliver integrated neighbourhood health services across Sussex.

Job responsibilities

The NHS Neighbourhood Health Plan sets out a shift toward delivering more care at home or closer to home, creating integrated, community-based services that improve access, experience and outcomes for people of all ages that support people holistically, recognising the interaction between physical health, mental health and social determinants of health.

It aims to build healthier, more independent communities by embedding joined up working between the NHS, local government, social care and voluntary partners as the norm rather than the exception. In Sussex we have formed the Neighbourhood Alliance to enable the delivery of this plan.

The Sussex Neighbourhood Alliance (NA) forms part of the wider Sussex Provider Collaborative (SPC), which brings together Sussex NHS provider organisations to jointly deliver large-scale transformation programmes across the system. The Neighbourhood Alliance is one of the Collaboratives core strategic alliances.

Job Purpose

The Neighbourhood Integrator is the dedicated integrator role for the Integrated Community Team (ICT) within the Sussex Neighbourhood Alliance. The post holder is accountable to the ICT Leadership Group for convening partners, coordinating delivery, and enabling the conditions for effective neighbourhood-level integration. The role provides consistent integrator capacity to ICTs at varying levels of maturity, ensuring progress towards a fully functioning neighbourhood health model.

A key expectation of the role is to actively promote and embed whole-person, integrated care, ensuring that mental and physical health services are brought together around the needs of individuals and communities. The Integrator will support the development of MDTs and neighbourhood approaches that address this holistically, with particular attention to reducing inequalities and the mortality gap experienced by people with serious mental illness.

The role is designed to enable, not burden, frontline services. A core expectation is that the Integrator reduces duplication, fragmentation and administrative overhead across the system, particularly for general practice and PCNs. The post holder will ensure that neighbourhood activity is coordinated and streamlined so that partners can participate effectively without additional unfunded workload.

The role does not replace the responsibilities of partner organisations or statutory leaders. It acts as a catalyst and enabling function, supporting collective ownership of outcomes and shared accountability for neighbourhood health.

Governance And Accountability

The Neighbourhood Integrator holds dual accountability:

  • Operational accountability to the ICT Leadership Group, which sets priorities and direction for neighbourhood delivery. The Integrator supports coordination, progress, risk management and delivery.
  • Programme accountability to the Sussex Neighbourhood Alliance and Sussex Provider Collaborative, including contribution to reporting, assurance, shared learning and alignment with system-wide priorities.

Key Responsibilities

  • Convening and Activation - Convene and support the effective functioning of the ICT Leadership Group, enable activation of all partners, build trusted relationships across a fragmented landscape, support shared decision-making and collective leadership.
  • Programme Oversight and Delivery - Hold local oversight of the ICT work programme, support establishment of MDTs for Highest and Ongoing Need, support prevention activity including vaccination uptake, drive continuous improvement and innovation, identify and escalate any system level barriers to integrated working via the appropriate route, ensure that programme delivery is designed to minimise additional workload for frontline services, and aligns with agreed capacity and funding.
  • Connecting to Place and Alliance Structures - Act as the primary link between neighbourhood delivery and Place and Sussex Neighbourhood Alliance structures, ensure alignment, shared learning and avoidance of duplication, develop and maintain a strong understanding of local and national imperatives for neighbourhoods and person-centred multi-agency working.
  • Asset Development - Develop and share understanding of local statutory and community assets, strengthen connections with VCFSE organisations, support asset-based approaches to neighbourhood health.
  • Data, Insight and Population Health - Support access to population health and performance data, enable proactive and preventative targeting, support use of ICT dashboards, translate data into shared intelligence, support identification of inequalities in outcomes between populations, including those with mental health conditions, and ensure this informs targeted interventions.

  • Significant experience working across health, care, local government or community systems
