opportunity has arisen for an experienced Care Coordinator to join the office team in Woking.
This is a rewarding opportunity for an organised, proactive and compassionate individual who is passionate about delivering outstanding care.
The successful candidate will play a key role in coordinating care services, supporting care staff and ensuring service users receive safe, responsive and person-centred care.
Key Responsibilities
- Coordinate daily care services to ensure safe, effective and high-quality care delivery.
- Produce, maintain and monitor staff rotas using the Care Management System (Care MIS), ensuring all visits are scheduled efficiently and service user needs are met.
- Manage electronic Medication Administration Records (e MAR) and support medication compliance.
- Create, review and update care plans, risk assessments and service user records.
- Monitor visit completion, respond to alerts and manage changes to care schedules.
- Supervise and support Care Assistants through spot checks, supervisions and performance reviews.
- Work closely with the Recruitment Team to maintain safe staffing levels and support new employee onboarding.
- Respond promptly to changes in care needs, emergencies and service issues, arranging cover where required.
- Participate in the out-of-hours on-call rota.
- Maintain accurate digital records and support compliance with CQC regulations, safeguarding requirements and company policies.
- Build positive working relationships with service users, families, healthcare professionals and colleagues.
- Contribute to continuous quality improvement and service development.
About You
- Previous experience as a Care Coordinator within domiciliary or community care.
- Practical experience using a Care Management System (Care MIS) for rostering, scheduling, care planning and electronic records.
- Experience using electronic Medication Administration Records (e MAR).
- Good knowledge of CQC Fundamental Standards and domiciliary care regulations.
- Excellent organisational, communication and problem-solving skills.
- The ability to manage competing priorities within a busy office environment.
- Good IT skills, including Microsoft 365 (Outlook, Word and Excel).
- A flexible approach, including participation in the on-call rota.
- A full UK driving licence and access to a vehicle.
- NVQ Level 3 in Health & Social Care (or willingness to work towards it).
