Reablement Support Worker
Location: Swindon
Client Group: Vulnerable Adults
Shift Pattern & Hours: Flexible rota covering days, evenings, weekends and bank holidays

We are currently seeking Reablement Support Workers to support vulnerable adults in their own homes, helping them regain and maintain their independence and develop everyday living skills.

This is a rewarding role for someone who enjoys supporting people to achieve greater independence while providing personal care and practical support when required.

Key

Responsibilities: Provide personal care and support with daily living activities.

Promote independence and help service users develop everyday living skills.

Support with mobility, transfers, medication and rehabilitation activities where required.

Monitor wellbeing and progress, reporting any concerns appropriately.

Maintain accurate and confidential records.

Work alongside families, carers and other health and social care professionals.

Follow safeguarding, health and safety and infection control procedures.

Requirements: Experience supporting vulnerable adults in a health or social care setting.

Experience providing personal care and supporting mobility.

Understanding of reablement and promoting independence.

Good communication skills and the ability to work with people with different communication needs.

Ability to work independently and as part of a wider team.

NVQ Level 2 in Care, or willingness to work towards this.

Ability to maintain accurate and confidential records.

Flexibility to work days, evenings, weekends and bank holidays.

Ability to travel within the required service area.
