As a Social Worker, you can be the reason why lives change for the better, helping some of the most vulnerable people gain a brighter future. With previous experience from the NHS or private care, you can join the multidisciplinary team on a part-time basis for 22.5 hours at Aderyn, which has a strong emphasis on teamwork and offers a high-quality step down service for men with enduring mental health conditions.

As a Social Worker, you will manage a complex caseload and be responsible for social care assessments, risk assessments, facilitate group activities, and contribute to ICR meetings, supporting service users to make viable plans for their future. You will report to the Safeguarding Lead, manage internal investigations, and liaise with home authorities for service users during their admission and discharging process, while enjoying access to a range of training opportunities that can help your career grow.

As a Social Worker, you will be:

  • Referrals to LADO and Local Authority in line with safeguarding and assessments
  • Educating Hospital staff about relevant safeguarding matters
  • Completing a full annual social care reassessment within the CPA format
  • Facilitating access to welfare rights, housing, advocacy services and associated support services
  • Participating in 1:1 supervision

To be successful in this role, you'll need:

  • A Social Work degree
  • Registered with Social Work England or applied for
  • Experience within a similar environment
  • Experience working with vulnerable adults

Where you will be working:

Location: Penperlleni, Nr Pontypool, Monmouthshire, Wales NP4 0AH. You will be working at Aderyn, a high-quality step down service for men with enduring mental health conditions. The unit is situated in a picturesque part of Wales with good transport links. You will work alongside a multidisciplinary team to provide service users with opportunities to improve their independence and prepare for an independent life in the community.
