We are searching for a Clinical Service Lead Nurse to join an award-winning community complex care provider based in Burton upon Trent. In this Complex Care Clinical Lead role, you will have the opportunity to lead the delivery of personalised care packages in the community, support a team of carers, and make a real impact on the lives of adults with complex health needs. You will be joining a highly-regarded, growing team that is tailored to ensure they provide the highest standards of clinical care.
As a Complex Care Clinical Lead you will:
- Create and manage tailored care plans for adult clients with complex care needs
- Lead, train, and support a team of carers to maintain high standards of care
- Utilise complex clinical skills including tracheostomy care, ventilator care and seizure management
- Work in partnership with CHC nurses, commissioners, and wider MDT to maintain bespoke care packages
- Build strong and lasting relationships with service users and their families
Criteria to apply:
- Registered Nurse (RGN) with an active NMC PIN
- Fully competent in tracheostomy & ventilator care
- Full UK driving licence
- Have a prior background in a similar clinical setting
- Good understanding of CQC standards & clinical governance
