As one of our Support Workers (Navigator), you'll play a key role in bringing that mission to life.
Hours of Work: 35 hours working Monday to Friday 9am to 5pm
Location: Leeds (Applicants should live in the local area. Access to your own vehicle is required due to travel requirements as part of the role. Mileage is paid within our company policy. You will also require business insurance.)
Salary: Starting salary £27,783.00 Per Annum
Contract Type: Full Time, Permanent

Please note: Inductions are held at our head office in Leeds on a Wednesday or Friday and travel expenses will be reimbursed for this.
The Ideal Support Worker (Navigator):
- Experience in support, outreach, customer-facing or community roles, such as housing, homelessness services, care, education, youth work, family support, criminal justice, retail or advice services.
- Able to build trust and positive relationships with people who may have complex needs, a history of rough sleeping, or difficulty engaging with services.
- Experience of supporting vulnerable people, or a good understanding of issues such as homelessness, social exclusion, poor mental health, substance use, offending, trauma and financial hardship.
- Able to use empathy, patience, professional boundaries and a person-centred approach to support people towards independence and tenancy sustainment.
- Confident working independently, managing a small but intensive caseload, keeping accurate records and contributing to multi-agency support plans.
- A full UK driving licence and access to your own roadworthy vehicle for work is essential.
What you will do as a Support Worker (Navigator):
- Client Guidance and Access to Services: Support people to access key services and practical support, including housing, health, benefits, employment, training, volunteering, banking and local community services.
- Client Integration and Personal Development: Help clients build confidence, resilience, independence, positive social networks and the skills needed to sustain accommodation and avoid returning to rough sleeping.
- Assessment, Documentation and Administrative Support: Carry out assessments, complete agreed assessment tools, maintain accurate records, monitor progress, collate case studies and contribute to project evaluation and reporting.
- Regular Support and Welfare Monitoring: Provide assertive, persistent and flexible support through regular contact, support sessions, appointments, reviews and welfare checks, reducing support gradually as independence increases.
- Partnership Working and Service Coordination: Work closely with voluntary and statutory agencies, landlords, housing teams and personal support networks to develop and review person-centred, multi-agency support plans.
- Risk Management, Behaviour and Professional Standards: Identify, assess and manage risk, including working with people who may display challenging behaviour or have a history of offending, while maintaining confidentiality, professional boundaries and high standards of practice.
- Team Contribution and Organisational Responsibilities: Work as part of a multidisciplinary team, share knowledge of local services, take part in training and partnership activity, act as an ambassador for the project and comply with Bridge-It Housing policies and procedures.
