We're currently seeking dedicated care assistants to join our team and help us continue providing outstanding in-home support.
We're recruiting Support Workers to support our Discharge to Assess (D2A) / Reablement service across Herefordshire. This role supports people returning home from hospital to recover safely and regain independence doing with, not doing for.

What you'll be doing:- Supporting people at home after discharge with reablement-focused care.
- Assisting with personal care where needed, while encouraging independence.
- Supporting daily living skills (washing, dressing, meal prep, mobility, routines).
- Following care plans and recording notes accurately.
- Reporting changes in condition, concerns, or risks promptly.
- Working respectfully with families and professionals as part of a wider team.
- Caring, reliable and professional approach.
- Experience in home care is helpful, but not essential (full training provided).
- Good communication and respect for dignity, choice and independence.
- Full UK driving licence and access to a vehicle (essential for Herefordshire).
- Right to work in the UK.
