About the Role
As our Complex Care Coordinator, you'll be the hub of daily care delivery, rostering visits, supporting care staff, liaising with health and social care professionals, and keeping real-time oversight of calls and records. This is a hands-on role where your attention to detail makes a real difference to people's lives.
What you'll be doing
- Building accurate roster and allocating visits in line with care plans, risk assessments, continuity and preferences
- Monitoring calls on Access and responding quickly to missed, late or short calls
- Coordinating same-day cover for sickness and emergencies and communicating changes clearly
- Checking daily care logs and medication records/MARs, escalating issues immediately
- Supporting quality and compliance through record checks, spot checks and staff supervision
- Liaising with GPs, District Nurses, pharmacies, social workers and Local Authority teams
- Escalating safeguarding concerns promptly and following local procedures
- Helping with assessments, care plan updates, reviews and settling-in checks for new packages

- Includes participation in an on-call roster.
What we're looking for (essential)
- Experience coordinating domiciliary/home care (or a similar operational role) - Minimum 2 years experience.
- Confident IT skills (Microsoft 365 + Access rota system, e-rostering/e-call monitoring systems such as Access)
- Knowledge of CQC Fundamental Standards / relevant domiciliary care requirements
- Strong organisation, communication and time-management able to work under pressure
- NVQ/QCF Level 2+ in Health & Social Care
- Experience supervising staff, rota management and completing spot checks/supervisions
