An opportunity has arisen for a clinical pharmacist to join South Uttlesford PCN, to provide high quality patient focussed pharmacy services to the patients of the PCN. This position is offered on a full-time basis and based in/out of a variety of South Uttlesford practices/locations according to need.

Responsibilities

  • Patient facing long‑term condition clinics to review the ongoing need for each medication
  • Patient facing clinical medication reviews
  • Patient facing care home medication reviews
  • Patient facing and telephone medicines support
  • Unplanned hospital admissions – review of medications and reconciling of medications
  • Repeat prescribing
  • Identification of high risk patients

Qualifications and Skills

  • Qualified pharmacist with strong experience within primary care
  • Minimum: hold a degree in Pharmacy (Masters or equivalent)
  • Registered with the General Pharmaceutical Council and a member of the Royal Pharmaceutical Society
  • Ideally a non‑medical independent prescriber (PCN will support attainment if not already)
  • Lead by example, setting and maintaining high standards of professionalism within a team environment
  • IT literate and confident in using practice systems

We formed our primary care network, to work together to enable us to operate more efficiently and ensure that a greater proportion of our resource is focused on clinical care and improved access. Our PCN covers 4 practices across West Essex: Peacock Surgery, Angel Lane Surgery, John Tasker House, supporting 54,000 patients.
