Responsibilities of this role:
- Working alongside the Care Manager to manage and support the running of the Reablement in Fleet and surrounding areas.
- Signing up new clients and liaising with Social Services and/or family members.
- Completing client reviews and ensuring our service is running smoothly.
- Creating and maintaining person-centred care plans.
- Carry out risk assessments within the service users' home.
- Communicate with our care staff regarding our client's needs and if any changes occur.
- Completing spot checks and face-to-face supervisions with our care staff and ensuring all concerns, if any, are reported to the Registered Manager.
- Compliance sign-off / medication competencies for new care staff.
