An All Care Registered Nurse redefines the standard of patient-centered care for patients while keeping them out of the crowded hospital system so they can heal and age where they are most comfortable – in their homes and community. Our nurses provide effective and sustainable care by meeting patients where they are. We design and deliver individualized care plans and exceptional clinical outcomes to our neighbors most in need. Schedule: Monday-Friday, 8am-4pm
Work Location: On the road in assigned territory, including Cambridge and surrounding communities
Position Type: Full Time
Responsibilities:
- Practice independently in the community as part of an interdisciplinary care team.
- Conduct comprehensive assessment of patients by determining patient needs, develops courses of action including personalized treatment plan and exceptional outcomes.
- Constantly evaluate evolving patient needs and respond with plan of care adjustments.
- Document all care provided and maintain records per All Care policy and standards.
- Participate in All Care team meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews.
- Travel to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
Qualifications:
Education: Bachelor’s degree in Nursing (BSN) or Associate’s degree in Nursing (ADN) from an accredited program. Current and valid Registered Nurse (RN) licensure in Massachusetts.
Experience: Previous nursing experience in a hospital, clinical, or home health care setting is required. Experience in specialty areas such as wound care, chronic disease management, or geriatric care preferred.
Skills: Strong clinical assessment and critical thinking skills. Ability to work independently and manage patient care in an unstructured, home-based environment. Proficiency in using Electronic Health Records (EHR) and other health care documentation systems. Knowledge of home health care regulations, care protocols, and best practices. Excellent communication, interpersonal, and organizational skills.
