Responsibilities
- Work alongside the Care Manager to manage and support the running of the Reablement in Fleet and surrounding areas.
- Sign up new clients and liaise with Social Services and/or family members.
- Complete client reviews and ensure the service is running smoothly.
- Create and maintain person‑centred care plans.
- Carry out risk assessments within the service user’s home.
- Communicate with care staff regarding client needs and any changes.
- Conduct spot checks and face‑to‑face supervisions with care staff, reporting any concerns to the Registered Manager.
- Complete compliance sign‑off and medication competency checks for new care staff.
Qualifications
- Experience within the care sector; willingness to obtain or currently hold NVQ level 2.
