Your Opportunity Integrated Home Care uses a strength–based Client Centered Case Management practice framework. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization.

You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment. Client care in a Home Care setting uses a collaborative, team-based approach while working remotely in a community setting. There is also an expectation of being a mobile worker. This role requires travel within the city of Airdrie and surrounding rural area, including caring for clients in their homes or in the community.

You will be responsible for operating a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and downstairs throughout the day is common within this job. Learning resources and training are provided to safely perform work.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications

  • Completion of an accredited nursing education program.
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
  • If you have not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses is required prior to employment:

Additional Required Qualifications

  • 2-5 years of recent acute care experience.
  • Previous Home Care experience or Case Management experience.
  • Experience with surgical wounds, drains, CVAD, HPTP, Palliative and end of life.
  • Experience working with acute clients and/or long-term clients.
  • Experience in providing case management services.
  • Recent experience in a community-based setting, Home Care experience.
  • Canadian Nurses Association (CNA) certification in Gerontology.
  • Experience in Home Parenteral Therapy (HPT), wound/skin and ostomy care, palliative support and client teaching care experience.

Preferred Qualifications

  • Bachelor’s degree in nursing.
  • Previous Home care experience and community care experience.
