Community Partnership Connector

The Community Partnership Connector is responsible for developing, coordinating and maintaining a diverse network of community, health, voluntary and statutory partnerships across the Contract Package Area (CPA) that enhance the support available to participants within the Confident Futures programme. Working closely with the Service Delivery Manager and the multidisciplinary delivery team, the postholder will identify, develop and promote community-based opportunities that improve participants' health, wellbeing, confidence, independence and social inclusion. Through partnership development, social prescribing and community engagement, they will help reduce barriers to participation and create sustainable pathways that support individuals to progress at a pace that is right for them. The postholder will maintain a comprehensive Community Asset Directory, strengthen referral pathways with local organisations, and ensure Health and Employment Specialists have access to high-quality, up-to-date information on local services and opportunities that complement personalised Support Plans. They will work collaboratively with partners across the health, voluntary, community and public sectors to maximise participant engagement, improve access to community resources and contribute to positive participant outcomes. The Community Partnership Connector will act as the bridge between participants, local communities and partner organisations, ensuring that every participant has access to the right support, in the right place, at the right time.

Key Responsibilities

• Develop and maintain effective partnerships with health, community, voluntary and statutory organisations to improve participant access to local support.
• Develop strong relationships with Social Prescribing Link Workers, Primary Care Networks, GP practices and Integrated Care System partners to strengthen referral pathways and maximise participant access to community-based health and wellbeing support.
• Create and maintain a comprehensive directory of local community assets, services and referral pathways.
• Promote social prescribing and community-based interventions that improve wellbeing, confidence, independence and social inclusion.
• Work collaboratively with Health and Employment Specialists to identify appropriate community opportunities aligned to participants' personalised Support Plans.
• Develop new community initiatives and wellbeing activities that address gaps in local provision and support participant progression.
• Organise and coordinate community engagement events, wellbeing activities and partnership forums that strengthen local collaboration.
• Maintain accurate records of partnership activity, participant referrals and community outcomes, contributing to contractual reporting and continuous improvement.
• Promote the Social Model of Disability by identifying and reducing barriers that prevent participants accessing opportunities within their local communities.

Essential/Desirable Skills, Knowledge and Experience

• Experience developing and maintaining effective relationships with community organisations, voluntary sector partners, local authorities, health services, employers and other key stakeholders.
• Good knowledge of local community services, health and wellbeing provision, social prescribing, and support networks across the Contract Package Area (CPA), or the ability to develop this knowledge quickly.
• Experience of working alongside, or a sound understanding of, Social Prescribing Link Workers, Primary Care Networks (PCNs) and community-based health and wellbeing services.
• Understanding of the principles of social prescribing and how community-based interventions can improve health, wellbeing, independence and progression towards employment.
• Excellent networking, communication and stakeholder engagement skills, with the confidence to develop productive partnerships at all levels.
• Experience working within community development, employability, housing, health, wellbeing, public health or voluntary sector services.
• Strong organisational skills, with the ability to coordinate multiple partnerships, maintain accurate records and manage competing priorities.
• Understanding of the Social Model of Disability and the wider determinants of health, with the ability to identify and reduce barriers to participation and inclusion.
• Willingness and ability to travel extensively across the Contract Package Area (CPA) to develop partnerships, attend meetings and support participants to access community opportunities.
