About AIOS
AIOS is building the world's first full-stack AI doctor. We're at $350M ARR, growing from $10M/yr 12 months ago. We're the world's fastest growing AI doctor, faithfully serving over 150k patients per month via Bolt Pharmacy, our main UK brand. We're profitable.
Our strategy exists at the intersection of two strong theses:
- The AI doctor that wins will get to escape velocity using GLP-1s, the fastest growing consumer product in history.
- The $2T European healthcare market is overlooked by the most talented builders.
Our master plan:
- Step 0 → $100M/yr by end of 2025: We went from $10M to $100M in 6 months serving the UK GLP-1 market.
- Step 1 → $1B ARR by end of 2026: Over the last 12 months we've grown from 3k to 150k UK active GLP-1 patients.
- Step 2 → $10B ARR by end of 2028: Blitzscale Europe.
- Step 3 → $100B/yr by end of 2031: Get regulatory approval across Europe for our Full Autonomous Prescribing (FAP) system.
- Step 4 → $1T/yr by end of 2035: With one line of code and zero human time, you can use AIOS to treat any patient globally with any medication.
We're a young, founder-led company. This is still Day 1 and all our work is ahead of us.
Being a Telehealth Medical Doctor at AIOS
We're building a world-class team. This is a part-time, sessional role — self-employed or contractor — with 5 to 10 sessions a week, flexible including evenings and weekends. You'll run synchronous 15 minute video consults, so you'll need to be online and available for the slots you book, rather than working asynchronously.
You'll report directly to Saim Dalvi, Chief Clinical Strategy Officer, and work under the clinical direction of Prof Frank Joseph, Medical Lead. We're intense, so you'll be too.
Key Responsibilities
- Video assessment: Run 15 minute synchronous video consults to protocol for BMI 25-27 patients, starting on time as booked.
- Documentation: Write notes that make the decision defensible and let another clinician take over the follow-up without needing to ask.
- Independent decisions: Prescribe, redirect or decline on clinical merit.
- Off-label governance: Hold consent conversations and produce documentation that meets GMC good practice on unlicensed and off-label prescribing.
- Verification: Blend shared decision making with independent verification where possible.
- Red flags: Screen for eating disorder risk, pregnancy and contraindications, and escalate within 24 hours.
- Audit and peer review: Participate in case review and act on audit findings.
- Feedback loop: Surface edge cases and protocol gaps to the Medical Lead and Clinical Product team.
KPIs
- Credentialed and live: GMC, DBS, induction and ClinicOS protocol sign-off.
- On time, every time: consults held to when they're booked, zero missed appointments.
- Throughput with quality: 15 minute video consults held to time, documentation audit pass rate 95%+.
- Defensible documentation: notes robust enough that any clinician picking up the follow-up can take over without asking questions.
- Decision quality: prescriptions in the 25-27 cohort carry a documented comorbidity, an off-label rationale and recorded consent, every time.
- Escalation discipline: red flags escalated within 24 hours through the escalation pathway.
Need to Have
- Registration: Full GMC registration with licence to practise, on the GP Register, revalidation current, clean.
- Prescribing Knowledge: Working fluency in off-label prescribing obligations (GMC Good practice in prescribing and managing medicines).
- Telehealth Experience: Telehealth or remote consultation experience.
- Documentation: Excellent documentation skills — robust notes that make the decision defensible and easy for another clinician to pick up and take over.
- Reliability: Consults held on time as booked.
- Eligibility: UK-based with right to work, enhanced DBS.
- Availability: 5-10 sessions per week including some evenings and weekends.
- Safety Mindset: Demonstrated comfort declining a paying patient and documenting why.
Nice to Have
- GLP-1 Experience: GLP-1 prescribing experience or structured weight management exposure.
- Specialist Training: SCOPE certification or other formal obesity medicine training.
- Digital-first: Time at another leading digital-first provider.
- Governance Experience: Audit or QI leadership.
- Startup Pace: Comfortable with a protocol that iterates monthly.
Compensation & Benefits
You get paid above market rate and you get early stage equity, so you get really rich if we nail this.
Compensation: £75 per hour plus £15 per hour in equity. Paid per session, never per prescription.
Benefits
- Training: initial training provided on protocols, ClinicOS and the pathway.
- Indemnity: provided by Bolt.
- Appraisal support: audit data, case mix summary and complaints record supplied for your NHS appraisal folder.
- Remote: fully remote video clinics, sessions scheduled around existing commitments.
