About the Role
We are currently seeking a dedicated and experienced Team Leader / Rehabilitation Support Worker to support an older female client living in the TN13 area. This is a highly rewarding opportunity to work within a dynamic multidisciplinary team (MDT), supporting a young individual with complex needs through a structured rehabilitation pathway. The role focuses on neurorehabilitation and spinal injury support, bridging the transition from hospital to home. You will play a key role in delivering tailored care plans, promoting recovery, and enhancing the client's independence and quality of life.
Key Responsibilities for Support Worker
- Deliver high-quality, person-centred care in the client's home
- Support rehabilitation programmes and therapy goals
- Assist with manual handling and mobility support
- Administer and monitor medication safely
- Work collaboratively with healthcare professionals and therapists
- Monitor progress and report changes in condition
- Provide emotional and psychological support to the client
- Help prevent complications and promote long-term wellbeing
Person Specification
Essential Skills & Experience
- Experience supporting individuals with spinal cord injuries and complex needs
- Strong manual handling skills
- Experience in medication administration and monitoring (hospital/ICU background desirable)
- Understanding of rehabilitation and recovery pathways
- Excellent communication and interpersonal skills
- Good IT and documentation skills
- Ability to work independently within a client's home
- Willingness to learn, adapt, and take feedback
