About the Role
We are currently seeking a dedicated and experienced Rehabilitation Support Worker to support an older male client living in the DA1 area. This is a highly rewarding opportunity to work within a dynamic multidisciplinary team (MDT), supporting a young individual with complex needs through a structured rehabilitation pathway. The role focuses on neurorehabilitation and spinal injury support, bridging the transition from hospital to home. You will play a key role in delivering tailored care plans, promoting recovery, and enhancing the client’s independence and quality of life.
