As an Outreach Support Worker, you will provide high-quality, person-centred support to individuals with Learning Disabilities, Autism, Mental Health needs, and complex support requirements, helping them achieve their personal goals, develop life skills, and maintain meaningful connections within their community.
Where you'll be working: Bromley, Bexley & Newham.
You will provide outreach support to individuals within their own homes and local community.

As part of your day to day work you will be offering the following;
- Supporting β Activities of daily living including social activities, personal care, routines, planning and preparation of meals and budgeting.
- Communicating β effectively communicate using people's chosen methods.
- Engaging β promoting and motivating people we support to enhance their quality of life.
- Specialising β supporting people who may need positive behavioural support approaches.
- Recording β keeping accurate records &reporting changes in health or circumstances of service users to management.
- Encouraging β people to play active roles in the communities in which they live, to encourage independence.

What is required from you?
