You will help two individuals with learning disabilities to thrive and live as independently as possible, providing person-centred support, personal care, and daily routines while building meaningful, lasting connections in the community.
You will encourage activities, support communication in a way that works for them, follow care plans, maintain accurate records, and promote safety and wellbeing.
