What is required from you?
- A minimum of 12 months on your right to work in the UK
- Great level of attention to detail and well organised
- Must be available to work 40 hours per - Office Hours - 08.30am - 5pm
- You will be part of the on-call rota system during evenings and weekends as and when required.
A day in the life of a Care Coordinator
As a Care Coordinator, you'll play a key role in making sure our clients receive consistent, high-quality care. You'll be responsible for matching and scheduling care workers with clients across the South West, while supporting new client starts and ensuring care arrangements run smoothly.
Your responsibilities will include:
- Planning and coordinating care schedules, ensuring they are delivered consistently and provide continuity of care for our clients.
- Supporting new client starts, including matching clients with suitable care workers and coordinating their schedules.
- Maintaining a clear understanding of safeguarding procedures and responding appropriately to any concerns, allegations of abuse or complaints relating to service delivery.
- Prioritising communication with next of kin, GPs, nurses, physiotherapists and other professionals where this may affect a client's care, health or wellbeing.
- Keeping the Registered Manager informed of relevant concerns, changes or developments.
- Ensuring all care worker holidays, sickness, unplanned absences and emergency calls are accurately recorded electronically.
- Working closely with care teams, clients and professionals to resolve scheduling issues and maintain a reliable service.
- Helping to ensure our clients receive the right care, at the right time, from the right person.
