We’re looking for a Care Coordinator at our New Milton Branch who enjoys organising, problem-solving, and connecting the right carers with the clients who need them most.

About you:

We’re looking for someone who is organised, people-focused and enjoys keeping things running smoothly, as well as:

  • Strong organisational skills and attention to detail
  • Ability to work confidently in a busy, fast-paced environment
  • A minimum of 1 years experience, or minimum level 2 NVQ in Health and Social Care
  • Full UK driving licence and access to your own vehicle
  • Minimum of 12 months remaining on your right to work in the UK

The role of a Care Coordinator:

  • Coordinate care services and client schedules
  • Match carers with clients to meet individual needs
  • Support the setup of new care packages
  • Plan rotas to ensure continuity of care
  • Communicate with families and healthcare professionals
  • Record staff availability, absences, and emergency calls
  • Participate in the on-call rota when required
  • Willingness to cover the on-call phone (additional pay provided)
