The Complex Care Adult & Paediatric Clinical Lead Nurse will provide strong clinical leadership across the South West region, ensuring the delivery of safe, high-quality, person-centred complex care services. The role involves supporting service growth, leading clinical practice, and working collaboratively with operational and management teams to mobilise and maintain care packages.
Key Responsibilities
- Clinical Leadership & Governance
- Provide clinical leadership, guidance, and ongoing support to Care Managers, Care Associates, and Healthcare Workers.
- Maintain high standards of clinical practice across adult and paediatric complex care services.
- Undertake and support clinical investigations, incident reviews, and learning outcomes where required.
- Ensure compliance with regulatory, safeguarding, and clinical governance standards.
- Care Planning & Service Delivery
- Assess new referrals, including comprehensive clinical assessments and risk assessments.
- Develop, implement, review, and audit individualised care plans.
- Support the mobilisation of complex care packages in collaboration with the Registered Manager and wider team.
- Review and reassess existing packages to ensure care remains appropriate and responsive to changing needs.
