Do you want a new role for the New Year working to improve quality of life and reduce poverty for older people in Sheffield? If you have drive, enthusiasm and a solution focused approach this position is for you Independent Living Coordinator (Hospital Discharge) 36.25 hours per week Hours to be worked flexibly Monday to Friday between 8am and 6pm £24,767 per annum, plus pension and 30 days annual leave plus public holidays This role will focus on supporting people who have had recent hospital admissions to help put everything in place to enable customers to continue to live independently at home You will be a people person, highly organised and skilled at practical problem-solving. You will be passionate about maximising older peoples income. You will relish working in a constantly evolving environment and enjoy working in the community, developing relationships with older people, GP Networks, hospitals, partner and referral organisations. You will facilitate access to services, resources and facilities, have excellent time management and IT skills and be comfortable managing a case load of customers and responding to high levels of demand for our services. You will receive a bespoke induction and training to equip you with the knowledge and skills to complete holistic assessments and provide solutions to identified issues. You will have access to shared learning sessions and external reflective practice sessions.
