Age UK Oxfordshire is seeking a Carer Adviser to work directly with unpaid adult carers, providing information, advice and practical support.
You will manage a caseload, coordinate with health, social care and voluntary sector partners, and help carers access services and maintain their wellbeing.
Working 24-35 hours per week, mostly home-based with travel across Oxfordshire, you will complete statutory Carer Assessments and develop personalised support plans in line with the Care Act 2014.
