As a Health Coach, you will promote health and wellbeing for individuals aged 50+ across Norwich and Norfolk. You will support clients at home, online, and in community settings to improve physical activity levels, build confidence, and access local services and social opportunities. The role is part of Age UK Norwich’s integrated community health service and contributes to preventative healthcare by enabling healthier, independent living. You will be part of the Service Delivery Team and report directly to the Service Delivery Lead.
Key Responsibilities
- Deliver tailored 1‑to‑1 health coaching via home visits, telephone, online platforms, or community‑based sessions.
- Facilitate group exercise and wellbeing activities to support physical and mental health goals.
- Encourage clients to commit to the sessions, setting goals and objectives to meet health outcomes.
- Support clients with building confidence, reducing isolation, and engaging in social and community networks.
Community Development
- Identify and empower ‘Community Champions’ to support peer‑led groups, share skills, and sustain engagement.
- Build and maintain relationships with community partners, health professionals, and stakeholders to enhance service reach.
- Represent Age UK Norwich at events and forums to promote our health and community support work.
Data & Evaluation
- Maintain accurate and confidential records of client engagement in line with GDPR and organisational policies.
- Conduct client feedback surveys and contribute to service evaluation and continuous improvement efforts.
General Duties
- Comply with Age UK Norwich’s Policies and Procedures.
- Prioritise client safety through ongoing risk assessments and timely reporting of concerns.
- Adhere to all regulatory and contractual standards.
- Commit to continuing professional development, staying current with evolving best practices and older people’s care needs.
- Undertake additional responsibilities reasonably requested by line management.
- Work flexibly as part of a multidisciplinary team, contributing to group sessions, peer learning activities, volunteer support, and cover during absences.
- Support continuous service development and delivery improvements alongside the Service Delivery Lead and Health Specialist.
- Help with outreach, education, and promotion by collaborating with the Marketing and Communications team across various platforms and community events.
Person Specification
- Experience delivering physical activity programmes for older adults (50+), especially those with long‑term health conditions.
- Ability to run both 1‑to‑1 and group exercise sessions across various platforms (in‑person and online).
- Strong understanding of health and safety, including contraindications to exercise and safeguarding protocols.
- Proficiency in Microsoft Office, data entry systems, and online meeting tools (e.g., Zoom, Teams).
- Excellent interpersonal, written, and verbal communication skills.
- Previous work within an Exercise Referral scheme.
- Experience engaging with individuals at risk of or living with frailty, social isolation, or moderate health conditions.
- Familiarity with working across multi‑agency settings (NHS, local authorities, voluntary sector).
Personal Attributes: personable and empathetic, confident to lead sessions, proactive problem‑solving, time and priority management, adaptable, innovative, and collaborative team member.
Qualifications
- Required: Level 3 Diploma in Exercise Referral or equivalent, or working towards.
- Preferred: Level 3 Adapting Exercise for Older Adults and/or Level 2 Chair‑Based Exercise.
- Current First Aid Certificate (or willingness to obtain).
Additional Information
- Role involves travel across Norwich/Norfolk – applicant must have own or public transport to work at multiple locations.
- Work flexibly in community settings including Age UK Norwich buildings, community buildings, GP surgeries, sheltered housing/care homes, and client homes.
- Full‑time 37 hours per week; hours flexible for post‑holder and charity needs, may include occasional evening and weekend work.
- Salaries paid monthly in arrears by BACS.
- Reasonable expenses reimbursed in line with policies.
- Full‑time annual leave entitlement is five working weeks, increasing with long service.
- Role subject to a Disclosure Barring Service (DBS) check.
