Age UK Lewisham and Southwark (AUKLS) is a local, independent, self-funded charity that exists to improve the lives of older people in Lewisham and Southwark. A key element of all roles is to develop and maintain the organisation’s working ethos and culture. As part of the social prescribing team, you will work in partnership with community organisations, health and social care and statutory services to provide a social prescribing service to people aged 60 and over, and/or self-identifying as a carer of an older person living in the London Borough of Southwark. The role will be based at our Stones End Day Centre, from partners' sites across the borough, in clients' homes and in other venues that clients might choose.

We are looking for a passionate person who enjoys a varied working day and is passionate about supporting older people to maintain their wellbeing and independence.
