Age UK Leicester Shire & Rutland are seeking a compassionate and proactive Outreach Memory Advisor to join our team on a fixed term contract. This role will focus on supporting individuals and families from African and Caribbean communities, working in partnership with the Communities Food & Wellbeing Hub.
The Outreach Memory Advisor will provide culturally sensitive information, guidance and practical support to individuals experiencing memory concerns and those affected by dementia. The postholder will play a key role in increasing awareness, reducing stigma, and improving access to services within African and Caribbean communities.
Working closely with the Communities Food & Wellbeing Hub, you will build trusted relationships, deliver outreach activities, and connect individuals to appropriate health, social care and community support services.
Key Responsibilities- Deliver culturally appropriate memory and dementia information and support
- Conduct outreach sessions within community settings.
- Work collaboratively with the Communities Food & Wellbeing Hub and partner organisations.
- Support individuals and carers with advice, signposting and advocacy.
- Increase awareness and understanding of memory concerns and dementia.
- Maintain accurate records and contribute to monitoring and evaluation.
