Providing essential support to patients over the age of 18 who are both at risk of admission and have been discharged from hospital, the Living Well Support Service:

  • Take patients home directly from the hospital and settle them back at home.
  • Provide continued support for up to 6 weeks, supporting people to remain independent.
  • Provide support to those individuals who are at risk of admission.

Based at Padiham Town Hall with some expectation of travel to Royal Blackburn or Burnley General Hospital sites, you will provide support to the team with data management and day-to-day administration, including contributing to the preparation of monthly and quarterly reports.

Qualifications

  • Excellent communication skills, both written and verbal.
  • Strong organisational skills and administrative role experience.
  • Ability to manage and prioritise multiple tasks to meet deadlines with a flexible approach.
  • Proficient in IT tools including the use of Microsoft Office and CRM systems.
  • Attention to detail especially when handling sensitive information, service and customer records.
  • Excellent interpersonal skills and ability to build relationships internally and externally.
  • Problem‑solving skills and the ability to suggest creative solutions to challenges.
  • Able to work within a team, take direction from others and collaborate effectively.
  • Experience of data collection and maintaining accurate records.
  • A positive approach and a genuine interest in supporting older people and understanding their needs.

This is a hugely rewarding role for the right person. The successful applicant will join a highly motivated and dedicated team that provides invaluable support to vulnerable or older people in the community after being discharged from hospital.
