Role Overview
We are seeking an additional team member to join our Gateway Outreach team, who supports adults through person‑centred, strengths‑based conversations focused on “What Matters” to the individual.
Responsibilities
- Use the My CAW assessment to co‑produce Personalised Support Plans promoting self‑management of health and wellbeing and early intervention.
- Provide short‑term, timely support, empowering individuals to access appropriate statutory and voluntary sector services and regain control over their lives.
- Support individuals approaching the end of life in a non‑clinical way, addressing social, emotional, and practical needs.
- Build trust and confidence through interpersonal skills, empathy, and creative support.
- Participate as part of the Gateway Outreach team and collaborate with partners.
Qualifications & Requirements
- Strong interpersonal and empathetic skills.
- Good communication with adults, fostering trust.
- Available to travel independently; driving licence and access to a car essential.
- Enhanced DBS check required.
- Willingness to undertake occasional weekend, evening, and bank holiday support coverage with notice.
Place of Work
Based in Falmouth Community Hub, Killigrew Street, FALMOUTH, TR11 3QP. Travel is essential; a driving licence and access to a car required.
Hours of Work
37 hours a week, Monday to Friday, with the requirement to undertake, with notice, occasional weekend, evening and bank holiday support cover from time to time.
Duration
12 months fixed term contract.
DBS Requirement
Subject to an Enhanced DBS check.
