Summary
- Liaising with clients to complete initial introductions to the service and manage expectations
- Provide telephone support to clients who would like an alternative to face-to-face support and carry out regular well-being checks for those who choose to delay service.
- Ensure that accurate client records are kept for each element of service provision made through the service.
- Support engagement officers with onward referrals and research where further ‘added value’ support could be offered either through Age Cymru Dyfed’s other services – e.g., Information and Advice, Befriending Life Links, Digital Support, Practical Services, etc., or another service provider.
- Support mid-point assessments – review current support provisions and report any concerns to Engagement Officers
