Age Concern Hampshire are working in partnership with Southern Health NHS Foundation Trust to instil vital confidence, social and mobility skills to patients who are preparing to return home. This is achieved by ensuring that social and support activities are integrated and embedded throughout the rehabilitation process, followed by ongoing communication once patients have been discharged.
To assist Age Concern Hampshire to provide support opportunities and activities for older people who are transitioning between a stay in hospital and their return home. Working on one of two rehabilitation wards within a community hospital, you would be providing a variety of support activities to promote confidence, wellbeing and independence, so that patients are able to return home safely and comfortably.
The role involves:
- Approaching patients with a view to gaining an overview of their needs
- Recording all information elicited about the client at each visit
- Discussing and agreeing one or more activities you could share
- Carrying out those activities at prescribed times and locations
- Supporting the H2H Co-ordinators to ensure best practise and outcomes
- Ascertaining and documenting all signposting and referral opportunities that will support the patient once discharged home.
