We have an exciting opportunity for an enthusiastic and caring Hospital to Home Assistant Coordinator to join our hard‑working team at Petersfield Community Hospital, Petersfield. The Hospital to Home Service provides short‑term, person‑centred support to promote recovery and enable safe and timely discharge from hospital. This role will suit an individual who has a passion for forming professional relationships with patients and can communicate with them in a way that respects their views, autonomy and culture.

Working 20 hours per week (Mon‑Fri), your responsibilities would include:

  • Patient Support & Activities
    • Support patients to settle into the ward environment
    • Deliver or support delivery of meaningful activities (1:1 and group‑based)
    • Adapt activities to meet individual needs, abilities, and preferences
    • Support patients to maintain social contact (family, friends, phone/video)
    • Provide reassurance and help patients understand information shared by professionals
    • Contribute to patient wellbeing and experience during their stay
  • Discharge Support
    • Assist the Senior Coordinator and MDT in planning safe discharge
    • Help identify suitable community services, activities, and support
    • Support arrangements that enable a smooth transition home
    • Undertake post‑discharge follow‑up contact where appropriate
  • Volunteer Support
    • Support the coordination of ward‑based volunteers
    • Assist with volunteer onboarding, guidance, and day‑to‑day support
    • Ensure volunteers are working in line with ACH policies and ward procedures
  • Working with the MDT
    • Work collaboratively with ward staff and MDT members
    • Share relevant patient insights to support care and discharge decisions
    • Attend MDT discussions where appropriate (with Senior Coordinator guidance)
    • Work flexibly to support ward priorities
  • Community Engagement
    • Build awareness of local community services and resources
    • Support linking patients into ongoing community support post‑discharge
    • Develop relationships with local voluntary and community organisations
  • Administration & Data
    • Maintain accurate and timely records (including Charity Log)
    • Support collection of KPI and evaluation data
    • Assist with reporting requirements for the service
    • Ensure volunteer records are up to date
  • Service Delivery
    • Support delivery of the service in line with agreed outcomes and KPIs
    • Contribute to continuous improvement and service development
    • Ensure patient dignity, choice, and confidentiality are always upheld
  • Governance & Compliance
    • Adhere to Age Concern Hampshire and NHS policies
    • Follow procedures relating to:
    • Safeguarding
    • Data protection
    • Health & safety
  • Escalate any concerns appropriately
  • Training & Development
    • Participate in supervision, appraisal, and training
    • Attend relevant team and service meetings
    • Contribute to shared learning and best practice
