Contract Type: Fixed Term
Working Hours: 18.75 hours per week, including travel time (to work flexibly across Mon-Wed or Wed-Fri).
Required: A full UK driving licence with access to a vehicle and a willingness to travel across the area, mileage paid. Previous experience working with young people with learning disabilities and autism. Sponsorship is not available for this position.
We have a great and exciting opportunity for a part-time Keyworker to join our team within our Children and Young People’s service in Derbyshire, on a fixed-term contract until March 2027 (with a potential extension subject to contract agreement).
The Keyworker service is part of the NHS long-term plan to support a joined-up working and consistent approach for each child and young person with a learning disability and/or children and young people who are autistic, with the aim to help reduce children and young people in crisis and to prevent admission or re-admission to hospital, helping them to access the right help at the right time across complex systems.
Who will I be working with?
You will be supporting children and young people with a learning disability and/or children and young people who are autistic through transition between services, through key stages in their lives, and provide a single point of support to help make the connections between children, young people, families, carers, and the array of services and support available.
What does a Keyworker do?
You will be supporting and implementing change for children and young people with a learning disability and/or children and young people who are autistic or both. You will be committed to building trusting relationships with children, young people, and their families and passionate about making a real difference. As a Keyworker, you will play a pivotal role to ensure a child-centred and whole family approach to meet their needs, aspirations, and outcomes for the children and young people.
The Keyworking service will aim to ensure that children, young people, and their families get the help they need to navigate complex systems and are supported by someone that they have built a trusting rapport with.
