The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists and technicians in general practice. In this role they will be supported daily by a clinical team that includes an advanced clinical practitioner pharmacist. There is a clinical lead who will develop, manage and mentor them. The post holder will work as part of a multi-disciplinary team in a patient-facing role and will be a prescriber. The role is predominantly about prescription management, medication reviews, building relationships with stakeholders and implementing safe and effective working practices. There will also be the opportunity to mentor and develop less experienced colleagues.
Main duties of the job
- You will take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes and those with multiple long-term conditions.
- You will support the management of the practice’s repeat and acute prescription management processes, as part of The Prescribing Team.
- You will review medication requests, issuing and arranging the necessary reviews as needed.
- You will work to improve communication about medication-related issues between the practice and other care providers.
- You will develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.

Job responsibilities
- As a more experienced Pharmacist you will provide mentoring, training and development for more junior colleagues.
- To ensure implementation of safe and efficient repeat prescribing systems.
- You will be part of a professional clinical network and will have access to appropriate clinical supervision as outlined in the Network Contract DES guidance where appropriate.
- You will support patients to take their medications to get the best from them, reduce waste and promote self‑care.
- In your leadership role you will be supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.

Person Specification
Knowledge and Skills
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
- In‑depth therapeutic and clinical knowledge and understanding the principles of evidence‑based healthcare.
Qualifications
- Master of Pharmacy Degree
- GPhC registration
Experience
- Minimum of 3‑5 years experience working as a Pharmacist within General Practice

- Able to obtain, analyse and interpret complex technical information.
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Good communication skills: written, verbal and listening.
