We are looking for a warm, energetic and dedicated Complex Care Support Worker to support a young lady with complex needs who thrives on positive relationships, meaningful activities and a nurturing environment. This role is ideal for someone who is naturally caring, enjoys having fun, and is passionate about helping young people achieve their fullest potential. You will play a key role in promoting independence, emotional wellbeing and social development, while ensuring safety, dignity and high-quality personalised care. Working closely with families and a multidisciplinary team, you will help create opportunities that enrich daily life and support the young person to flourish.

Responsibilities

  • Delivering personalised care that meets physical, emotional and social needs in line with an individualised care plan
  • Supporting daily routines including personal care, mobility, meals, communication and structured activities
  • Enabling fun, confidence-building experiences at home and in the community, including outings, hobbies and social opportunities
  • Driving to appointments or activities when required to support participation and independence
  • Recording clear, accurate notes on progress, wellbeing and any changes in presentation
  • Working collaboratively with families, healthcare professionals and colleagues to maintain consistent, high-quality support
  • Maintaining a safe, nurturing environment through adherence to safeguarding and health and safety standards
  • Engaging in ongoing training and development to ensure best practice in complex care

Skills

  • Experience supporting young people with disabilities or complex needs
  • Excellent communication skills, including clear spoken English, to build trust with the young person and their family
  • Ability to follow detailed care plans and adapt support to changing needs
  • Confidence using IT systems for documentation and reporting
  • A full driving licence is desirable but not essential
  • A compassionate, patient and fun-loving approach, with the ability to create positive, engaging interactions
  • Strong organisational skills and the ability to manage varied responsibilities
  • Flexibility and resilience, with a commitment to supporting the young person's development and social outcomes
  • Desirable Polish speaking
  • Experience in day services with young adults

Work Location: In person. Must have access to own car and be willing to take the client in their own car during the day.
