CCH Group in Cardiff is seeking a Care Coordinator to organise and deliver high-quality home care. You will manage referrals, create rotas for care workers, liaise with social workers and GPs, and monitor service users’ conditions.
You’ll work Monday to Friday, 9am–5pm, with on-call duties on a rota. A driving licence is required, and you’ll join a team dedicated to compassionate, client‑centred care.
