We are seeking a dedicated and proactive Pathways and Support Coordinator to join our team. This is a vital role focused on making best use of floating support and supported housing for those homeless or rough sleeping. You will be at the forefront of our efforts to provide timely support to some of the most vulnerable members of our community, maintaining current partnerships and developing new ones.

About The Role

You will be the primary point of contact for the Housing Needs Team - homeless and outreach teams; support and supported housing providers. You will be responsible for assessing referrals from the service and making nominations into commissioned and non-commissioned provision - both floating support and housing.

The role will lead on forming relationships with non-commissioned providers and working with the Rough Sleeper Coordinator to develop a single persons pathway for those homeless and rough sleeping to bring commissioned and non-commissioned providers together to collaborate and provide support and supported housing with the aim to prevent homelessness and rough sleeping.

The successful candidate will be subject to a Standard DBS check clearance.
