Overview

This is reflected in the Councils\’ corporate plan – Our Plan. We are looking for a highly motivated and empathetic individual with a passion for making a positive impact on people\’s lives. As a Proactive Case Worker, you will be responsible for managing a caseload of individuals, providing comprehensive support and financial guidance to help them overcome challenges and achieve their goals. You will adopt a proactive approach, identifying needs, coordinating services, and building strong relationships with clients and external agencies.
Key Responsibilities
- Manage a diverse caseload, conducting assessments and developing tailored action plans.
- Provide ongoing support, advocacy, and signposting to appropriate services.
- Work collaboratively with clients to set achievable goals and monitor progress.
- Maintain accurate and up-to-date case notes and records.
- Collaborate effectively with internal teams and external partners to ensure holistic support.
- Participate in team meetings and contribute to service development.
- Proven experience in a case management or similar role.
- Excellent interpersonal and communication skills, with the ability to build rapport and trust.
- Strong organisational skills and the ability to manage a demanding workload efficiently. A proactive and solution-focused approach to problem-solving.
- Empathy, sensitivity, and a non-judgmental attitude.
- Ability to work independently and as part of a team.
- Proficiency in IT systems and record-keeping.
