We are seeking a dedicated and experienced Social Worker – Adults Services to join our client’s team on a contract basis. About the Role:

As a Social Worker – Adults Services, you will play a vital role in supporting adults within the Fylde & Wyre area. Your responsibilities will include:

  • Coordinating hospital discharges
  • Conducting assessments
  • Ensuring the best outcomes for service users

This position requires someone who is proactive, compassionate, and able to work independently.

Candidate Responsibilities:

  • Conduct Continuing Health Care Assessments and Reviews
  • Complete Mental Capacity Assessments in accordance with the MCA 2005
  • Apply eligibility criteria under the Care Act effectively
  • Manage hospital discharge pathways to ensure timely and safe discharges
  • Triage for hospital discharges via SPA (Single Point of Access)
  • Maintain accurate and detailed case records
  • Liaise with multidisciplinary teams, service users, and their families
  • Ensure compliance with relevant statutory and organizational policies

Candidate Skills & Requirements:

  • Ability to drive and access to personal vehicle is essential
  • Previous experience or knowledge of hospital discharge pathways
  • Strong understanding of the Care Act, MCA 2005, and adult social care processes
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage caseloads effectively
  • Immediate availability for interviews and start
