We are seeking a dedicated and experienced Social Worker – Adults Services to join our client’s team on a contract basis. About the Role:
As a Social Worker – Adults Services, you will play a vital role in supporting adults within the Fylde & Wyre area. Your responsibilities will include:
- Coordinating hospital discharges
- Conducting assessments
- Ensuring the best outcomes for service users
This position requires someone who is proactive, compassionate, and able to work independently.
Candidate Responsibilities:
- Conduct Continuing Health Care Assessments and Reviews
- Complete Mental Capacity Assessments in accordance with the MCA 2005
- Apply eligibility criteria under the Care Act effectively
- Manage hospital discharge pathways to ensure timely and safe discharges
- Triage for hospital discharges via SPA (Single Point of Access)
- Maintain accurate and detailed case records
- Liaise with multidisciplinary teams, service users, and their families
- Ensure compliance with relevant statutory and organizational policies
Candidate Skills & Requirements:
- Ability to drive and access to personal vehicle is essential
- Previous experience or knowledge of hospital discharge pathways
- Strong understanding of the Care Act, MCA 2005, and adult social care processes
- Excellent communication and interpersonal skills
- Ability to work independently and manage caseloads effectively
- Immediate availability for interviews and start
