The predicted salary is between 36000 - 60000 £ per year.
This role is on a temporary contract of 3–6 months, with the potential for a permanent position for the right candidate. The position (E17 4JF) is hybrid, requiring one day in office (Tuesdays) and four days working from home.
Responsibilities
- Coordinate the brokering of services on behalf of Adult Social Care for residents with social care needs, using an understanding of client groups, systems, care markets and negotiation skills to ensure that appropriate, value‑for‑money care is commissioned.
- Ensure referrals for discharges from hospital are facilitated in a responsive way to the targeted timeframes of the NHSE guidance (7 days a week, 8am – 5.30pm) and within 3 hours of a resident becoming optimised for discharge.
- Respond in a timely way to community referrals to source and purchase appropriate settings to meet the needs of the residents.
