Supported Assistant Practitioner/Assistant Practitioner 8862 | Fixed term contract until 21 September 2026 | £28,598 to £33,699 per annum (Scale F/G) | 37 hours per week | Greyfriars House, Great Yarmouth
An exciting secondment opportunity to join the Eastern Home First Team within the Eastern Locality to provide timely follow up assessment to people discharged from acute hospitals under Discharge to Assess (D2A) and the NHSE Hospital Discharge Service: Policy & Operating Model. Norfolk County Council aims to ensure adults discharged from James Paget University Hospital, other acute hospitals, and community short-term placements are supported to return home, access reablement opportunities and plan ongoing care and support needs.
The Homefirst Team works towards an integrated care system with partners including Health; Integrated Care Board, Eastern Locality Primary Care Hubs, Norfolk First Support and Caring for Better Outcomes providers. The team provides in-reach support with the Acute Hospital and follows up transfers of care within the Virtual Homefirst Hub, conducting Care Act Assessments and Carers assessments and reviews. This integrated approach offers variety and opportunities to build strong working relationships across county council, NHS and partner organisations.
Using the Living Well model, the role focuses on enabling early intervention in the community to improve outcomes and help people remain independent, resilient and well. The role involves managing your own diary with autonomy to decide on face-to-face visits, with flexible remote working and base options of office and homeworking. Our technology platform and equipment support remote collaboration.

Responsibilities
• Provide timely follow-up assessments for individuals discharged to the community under D2A and related hospital discharge services.
• Work within the Living Well model to promote independence, early intervention, and appropriate community support planning.
• Manage own diary with flexible working arrangements and use of remote technology to connect with health and care colleagues.
• Undertake Care Act assessments, Carers assessments and reviews in collaboration with partners as required.
• Contribute to integrated care initiatives across the Eastern Locality in partnership with NHS and local authorities.
Qualifications and Skills
• Either a Supported Assistant Practitioner (Scale F) or an experienced Assistant Practitioner (Scale G) with relevant qualifications and experience.
• Ability to complete Care Act assessments and collaborate with multi-disciplinary teams.
• Strong communication and relationship-building skills to work with a range of professionals and service users.
