We are pleased to announce an opportunity for a Social Worker with the Children with Disabilities Service, in our West and Breckland team.

Social Worker – Children with Disabilities | 8613 | permanent contract | 37 hours per week | Ā£39,862 to Ā£48,227 per annum | Grade J/K depending on experience | West Norfolk and Breckland

We are pleased to announce an opportunity for a Social Worker with the Children with Disabilities Service, in our West and Breckland team.

Our Children with Disabilities (CWD) service provides support to children and young people with substantial and permanent disabilities who are 0-18 years of age.

We have three teams which cover Norfolk, and they are multi-disciplinary consisting of Social Workers, Family Practitioners, Assistant Practitioners, Occupational Therapists and Occupational Therapist Assistant Practitioners.

The CWD West and Breckland team are a close-knit team working in a multi-disciplinary approach to support children, young people and their families across all aspects from initial referral to their full journey through Children’s Services including CIN, CP, CLA and transitioning to Adulthood.

We maintain strong links with internal and external partners to ensure a holistic response to the needs of the children, young people and families we are working with.

The Social Worker will be case holding, working to identify areas of need, identifying support packages that are appropriate to meet the identified needs and pieces of individual work through our CWD Occupational Therapists or CWD Outreach to enable young people have equal opportunities as those of their non-disabled peers.
