Overview
Integrated Care Coordinator (ICC) – 12 month temporary contract. £27,711 to £30,060 per annum (Scale F) | 37 hours per week | Reed House, Norwich.
An exciting opportunity within the Central Norfolk HomeFirst Hub Integrated Care Coordinator team for a full-time ICC on a temporary 12 month contract. The post operates on a rolling 4-week pattern and will include weekend work.
The HomeFirst Hub covers Norwich as well as North and South Norfolk and works with Norfolk Community Health & Care (NCH&C), NHS Acute colleagues, the Voluntary sector and other Norfolk County Council colleagues to support Discharge to Assess pathways, crisis support in usual places of residence, and aiming to prevent unnecessary admissions to hospitals or care homes.
You will plan and coordinate discharges from acute hospital settings to enable individuals to return home, access reablement opportunities, and make onward referrals using a holistic approach. You will collaborate with multi-agency teams including Nurses, Therapists, Care Providers and Social Workers.
Responsibilities
- Plan and coordinate discharges from acute hospital settings to support individuals returning home and accessing reablement opportunities.
- Apply a holistic approach to onward referrals and support for people in crisis in their usual place of residence.
- Monitor patient journeys by desktop analysis of records and telephone contact with individuals supported.
- Support colleagues to manage situations using data, system knowledge and onward referral pathways.
- Work with multiple systems (LAS, SystmOne, Optica, NCAN) and make onward referrals to external and voluntary organisations.
- Collaborate with Nurses, Therapists, Care Providers and Social Workers to promote multi-agency working.
Qualifications and Skills
- High level of education (e.g., NVQ level 3, GCSEs or equivalent).
- Experience in the voluntary sector, primary care, the NHS, Social Care or a multi-disciplinary team.
- Positive, flexible, and adaptable with an ability to work at pace in a busy environment.
- Awareness and understanding of relevant health and social care legislation; excellent interpersonal and organisational skills; strong administrative and IT skills.
- Willingness to work in a blended model of office and virtual working.
