Byddwch yn goruchwylio'r gwaith o ddarparu ein dau Wasanaeth Byw â Chymorth Cofrestredig AGC yn Sir Gaerfyrddin, gan gefnogi unigolion sy'n profi salwch meddwl difrifol ac anghenion cymhleth. Darperir y gefnogaeth gyda'r nod o gael holl dîm staff Adferiad i annog a galluogi cleientiaid i gyrraedd lefel uwch o annibyniaeth ac yna hwyluso eu symud ymlaen i fyw'n fwy annibynnol yn y gymuned.
