Peer Mentor in Colwyn Bay

Hours: 37.5

Salary: £26,689.16

Based at: Conwy

Role Purpose:

- Provide support and guidance to all participants that have been registered onto the project.

- To develop ongoing action plans with participants which will be reviewed on a regular basis.

- To provide one to one support and group activities to aid the development of a participant in

their recovery.

- Work alongside the team structure in delivering services and outcomes for the scheme.

- To achieve project targets and outcomes set by Welsh Government.

- To support the team leader and referral agents in raising awareness of the service.

- To work in partnership with other practitioners to deliver effective interventions and support for

people with Mental Health and/or substance misuse.

- Provide support and supervision to volunteers.

- Datblygu cynlluniau gweithredu parhaus gyda chyfranogwyr a fydd yn cael eu hadolygu'n

rheolaidd.

- Darparu cymorth un-i-un a gweithgareddau grŵp i gynorthwyo datblygiad cyfranogwr yn ei

adferiad.

- Gweithio ochr yn ochr â strwythur y tîm wrth ddarparu gwasanaethau a chanlyniadau ar gyfer y

cynllun.

- Cyrraedd targedau a chanlyniadau'r prosiect a osodwyd gan Lywodraeth Cymru.

- Cefnogi arweinydd y tîm a'r asiantau cyfeirio wrth godi ymwybyddiaeth o'r gwasanaeth.

- Gweithio mewn partneriaeth ag ymarferwyr eraill i ddarparu ymyriadau effeithiol a chymorth i

bobl â Iechyd Meddwl ac/neu camddefnydd sylweddau.

- Darparu cymorth a goruchwyliaeth i wirfoddolwyr.
