Young Persons Outreach Worker
Contract Type: Temporary
Hourly Rate: 18.85 per hour
Working Pattern: Full Time (37 hours per week)
Location: Wolverhampton (driving is essential as you are required to travel for visits to young people)
Job Purpose and Role:
As a Young Persons Outreach Worker, you will deliver a comprehensive Leaving Care Service, ensuring that young people leaving care receive the support and guidance they need. You will work in accordance with relevant legislation, including the Children (Leaving Care) Act 2000 and the Children Act 1989.
Your Key Responsibilities Will Include:
- Manage a Caseload: Provide tailored support to care leavers based on their individual Pathway Plans
- Collaborate Effectively: Work alongside social workers to co‑work with eligible children in care
- Pathway Planning: Help develop and regularly review Pathway Plans to ensure each young person's needs are met
- Support Transitions: Ensure care leavers have the resources needed for a successful transition to independence
- Engage Young People: Deliver engaging group sessions, activities and personal development programmes
- Stakeholder Liaison: Communicate with internal and external stakeholders to promote collaboration and access additional services
- Educational Monitoring: Support care leavers in achieving their educational goals, including further and higher education
- Record Keeping: Maintain accurate records and generate reports as needed
What We're Looking For:
- Relevant experience working with young people, especially those leaving care
- Strong knowledge of legislation and policies pertaining to young people in care
- Excellent communication and interpersonal skills to build rapport with young individuals
- A collaborative spirit, able to work within a multidisciplinary team
