Exciting Opportunity: Are you ready to make a difference in patient care? Our client is looking for a dedicated and enthusiastic Pathway Tracker to join their team on a temporary full-time basis asap to March 31, 2026. If you have a passion for healthcare and a knack for organisation, this role is perfect for you!

Key Responsibilities:

  • Patient Journey Management: Proactively track patients from referral to treatment, ensuring a seamless experience through efficient management of waiting lists and progress monitoring.
  • Target Monitoring: Ensure adherence to national targets, particularly the 18-week Referral to Treatment (RTT) standard. Validate data and identify potential breaches to maintain high standards of care.
  • Data & Reporting: Maintain accurate patient records using electronic systems (such as PAS), ensuring data quality for audits and reporting.
  • Liaison & Escalation: Collaborate closely with clinicians, secretaries, and administrative staff to resolve bottlenecks. Don’t hesitate to escalate issues when interventions are needed!
  • Coordination: Arrange diagnostic tests, appointments, and admissions to keep patient pathways moving smoothly.
  • Meeting Support: Facilitate Multi-Disciplinary Team (MDT) meetings, ensuring smooth operations and accurate recording of decisions.

Essential Skills & Experience:

  • Excellent organisational skills and meticulous attention to detail.
  • Strong communication and interpersonal skills to engage effectively with diverse teams.
  • Ability to work independently, prioritise tasks, and tackle problems with confidence.
  • Familiarity with electronic patient administration systems (PAS) and proficiency in MS Office (especially Excel).
  • Understanding of medical terminology and targets/guidance.
