Job Description:
- The Speech-Language Pathologist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.
- Evaluate patients and perform appropriate therapy interventions.
- Develop a comprehensive plan of care and coordinate multi-disciplinary care.
- Perform clinical documentation and visit notes.
- Continued communication with physician, care team members, patient and family.
- Comfortable working in all aspects of the home health setting.
- Implement and maintain SLP protocols.
- Manage patient treatment and outcomes directly.
Requirements:
- Current Texas Speech-Language Pathology License.
- One year of SLP experience.
- Experience with Homecare Homebase is helpful, but not required.
- Previous Home Health experience is helpful, but not required.
- Current Texas driver's license.
- CPR certification.
